Hospital bill check: the itemized bill against the EOB

General information, not legal, medical or financial advice. Ask a lawyer before acting on it.

How to read one line

An example line on an itemized bill: 06/03/2026 · 0450 · EMERGENCY ROOM · 1 · $1,950.00. That is the date, the revenue code, its description, the quantity and the charge. If the same line appears again with the same date, ask why.

Found an error?

List each line, its amount and what is wrong in the charge dispute letter. If the bill shows only totals, ask for the itemized bill first.

Questions people ask next

Where do I find the billing codes?

On the itemized bill. If your bill shows only totals, ask the billing office for the itemized bill in writing.

What is upcoding?

Billing a service under a code for a more complex or costly service than the one you got. If a code's description doesn't match what happened, list it in your dispute letter.

What if the bill came before my insurer's EOB?

Ask the billing office to hold the account until your insurer has processed the claim, then compare the two.

What do I do with the errors I find?

Dispute them in writing with the charge dispute letter, and keep a copy.
