Bill Pushback / Itemized bill

How to request an itemized medical bill

Fill in the letter, print it and mail it to the billing office. Keep a copy and note the date you sent it.

Your name, addresses and numbers stay in this page. Only the words in "In your own words" leave it, and only when you press Polish my wording: they go through our server to an AI that fixes spelling, grammar and tone, and are not saved. It adds no facts and no advice.

Re: Request for an itemized bill. Patient: [Patient name]. Account: [Account number]. Date of service: [Date of service].

To the billing office:

I received a bill for [amount] for the care listed above. Please send me a complete itemized bill that shows, for each charge:

  • the date of the service;
  • a description of the service, supply or drug;
  • its billing code (CPT, HCPCS or revenue code) and the quantity;
  • the amount charged;
  • every payment, insurance payment and adjustment applied to the account.

Please also tell me whether any discount or financial assistance policy applies to this account, and send me a copy of it.

While I review the itemized bill, please do not send this account to a collection agency. Please send the itemized bill to the address above.

Thank you,

General information, not legal, medical or financial advice. Ask a lawyer before acting on it.

Why ask for it

A summary bill shows a total, not what you are paying for. The itemized bill lists every charge with its date, code and amount, so you can check it against what happened and against your insurer's explanation of benefits (the EOB).

What to check when it arrives

  • The same charge listed twice, or a quantity that looks wrong.
  • A service, test or drug you did not receive.
  • Days billed after you went home.
  • Charges your insurer says it paid or the provider should have written off.
  • The patient name, insurance ID and dates.

Found something? Use the charge dispute letter.

If they don't answer

Call the billing office, ask for the itemized bill again, and write down the date, the name of the person you spoke to and what they said. Send the letter again by certified mail so you have a record that it arrived.
